Patients’ Right to Know Act: Service Availability Form
Colorado Law now requires health facilities to provide patients with information on the
availability of services related to:
- Reproductive health care
- LGBTQ health care services; and
- End-of-Life health care services
Colorado law requires health care facilities to complete a Service Availability Form and provide it to patients so they are aware of the services and/or referrals for services that we provide. In 2023 Governor Polis signed into law HB23-1218 which protects a person’s right to make informed choices about their health and consent to their medical treatment.
